Keep In Touch Program Volunteer Application

Thank you for your interest in the Keep in Touch program. The volunteer-based program fosters relationships between community members like yourself who want to serve, with those needing connection. Our goal is to pair our adult volunteers with seniors who are experiencing feelings of loneliness and isolation for weekly telephone calls.

As a Keep in Touch program volunteer, your role is to:

  • Phone your pairing each week at a pre identified time
  • Provide a reassuring, friendly and consistent voice to your pairing
  • Help connect your pairing to community resources and support to meet their needs
  • Act as an emotional, social and isolation support to your pairing

As a Keep in Touch Volunteer, the anticipated time commitment per pairing is:

  • 1 - 1.5 hours per week to connect with your pairing
  • 1 optional virtual team connect meeting per month 

Below is the list of you will need to complete the volunteer application form (this form can be saved and you can come back to it at any time using the "Save" button at the bottom of this page)

  • Complete this volunteer application Form
  • Submit a Government-issued piece of photo ID with your current address (there will be a place to upload a photo of your ID on this form)
  • Names and contact information, including an email address for 1 volunteer, work-related, or community reference and 1 personal character reference
  • Submit your resume (if available)

Once you complete the application, you will be required to:

  • Submit a criminal record check that is dated no more than 3 months prior to today's date (We will send you a letter to have this completed at a reduced rate via Email. Please keep your receipt and we will reimburse you for the cost of your criminal record check);
  • Send your references to the link (if applicable)
  • Complete a virtual interview with the Program Coordinator (this will be set up by our Program Coordinator)

Please note that all references must have known the applicant for at least one year and must meet the criteria outline below. Once you submit the application, you will be provided with a link to send to each of your references listed below. If you do not provide an email for your reference, the Program Coordinator will complete your reference checks by phone.
